A session map for coordinating medical questions, psychological goals, daily support, professional roles, and review criteria.
Consider using when
- the client has many recommendations but no practical sequence;
- several professionals are involved and responsibilities feel unclear.
Pause or refer when
- the therapist is expected to direct treatment outside scope;
- the plan is being used to avoid necessary reassessment.
Four stages of work

Medical priorities
Clarify active questions and treatment.
- What requires diagnosis, monitoring, or follow-up?
- Which prescriptions and restrictions are current?

Psychological goals
Choose a focused therapy target.
- Is the main target panic, avoidance, grief, overload, shame, or another process?
- How will progress be measured beyond symptom elimination?

Daily supports
Identify basic and social resources.
- Which sleep, food, movement, pacing, or support needs are most relevant?
- Who can help with practical load?

Coordination and review
Define roles and timing.
- Who is responsible for each next step?
- What change would require revising the plan?
Integration
Turn insight into a coordinated next step
Keep the plan small enough to use and flexible enough to revise.
- Keep the plan to a few concrete actions.
- Document role boundaries and consent for coordination.
- Set a specific review date.
Safety and scope
Professional boundaries
- The client may skip any question, change direction, or stop the exercise at any time.
- This session map does not diagnose a condition or prove that a physical symptom has a psychological cause.
- New, severe, or changing symptoms deserve medical assessment. Mental health work should not cancel prescribed treatment.
- These educational materials are not a substitute for professional judgment, supervision, local law, or scope-of-practice requirements.
