Insomnia, Evening Arousal, and the Struggle to Sleep

A session map for exploring sleep effort, evening activation, unfinished tasks, routines, and referral needs without promising immediate sleep.

Sleep and Recovery30–45 minutesDepth: moderateVersion 1.0
Consider using when
  • worry about sleep is increasing evening arousal;
  • the client spends more time in bed trying to force sleep.
Pause or refer when
  • severe daytime sleepiness creates immediate driving or workplace risk;
  • possible sleep apnea, medication effects, mania, or another condition has not been assessed.
Session route

Four stages of work

01

Sleep pattern

Map timing and variability.

  • What are the usual sleep and wake times?
  • What happens on difficult nights and the following day?
02

Arousal cycle

Identify effort and monitoring.

  • How does the client try to make sleep happen?
  • What does clock-watching or extending time in bed do to anxiety?
03

Evening transition

Find realistic cues for ending the day.

  • Which tasks or worries stay mentally open?
  • What low-stimulation routine could be repeated?
04

Support plan

Define treatment and safety needs.

  • Would structured CBT-I or medical evaluation be appropriate?
  • What small change can be tested for one week?
Integration

Turn insight into a coordinated next step

Keep the plan small enough to use and flexible enough to revise.

  1. Keep wake time reasonably consistent.
  2. Track only variables that support decisions.
  3. Refer when breathing, movement, mood, or medication concerns are present.
Safety and scope

Professional boundaries

  • The client may skip any question, change direction, or stop the exercise at any time.
  • This session map does not diagnose a condition or prove that a physical symptom has a psychological cause.
  • New, severe, or changing symptoms deserve medical assessment. Mental health work should not cancel prescribed treatment.
  • These educational materials are not a substitute for professional judgment, supervision, local law, or scope-of-practice requirements.
